Grant Forms

To apply for a grant:

File a Letter of Intent (below) at least 30 days prior to the grant application deadline.
If project is deemed eligible, file a full grant application by the deadline.

Once your project is completed, you must fill out an evaluation and return it to the Foundation. You have one year from the project start date to complete the evaluation.
